Investigations

A serious workplace incident can leave employees and managers who have been involved directly or indirectly with the event with tragic memories for many years after the event, particularly if the root cause(s) of the incident was never genuinely identified and no appropriate remedial action was taken. Unfortunately even though Inspectors investigating the incident may empathise with the situation, they are generally not visiting to comfort you. They are employed to obtain facts and identify breaches from the moment they enter a site. Any evidence collected may later be used to take legal action against your company or you as a PCBU or officer.
